Online Nutri-Physical
The Nutri-Physical is an online questionnaire designed by a team of physicians to determine the best nutritional program for you, based on your lifestyle and current health profile. You can take the Nutri-Physical any time at your convenience. The nutritional recommendations are completely optional. If you choose, you can customize a supplement formula just for you, based on your personalized recommendations.

Note: Your Nutri-Physical answers are completely private. They will be used by a sophisticated program to create a nutritional regimen recommendation. We will be notified of your nutritional recommendations solely for the purpose of providing guidance and answering any questions you may have.

Personal Consultation
You can submit a question or a request by email for a personal consultation with nutrition and wellness specialist, Jayna Locke. I will call or email you back to set up a time. In our consultation, I will ask questions about your health, habits, and diet, and provide specific lifestyle and nutrition recommendations. I can help you address health concerns such as chronic pain, migraines, cardiovascular health, blood sugar challenges, digestive disorders, skin problems, energy and vitality, weight challenges, and many other issues.

The wellness regimen I design for you may include a weight management program, nutritional supplements, exercise and lifestyle changes that can help to optimize your health, weight, and the energy and mental health you experience each day. All recommendations are optional. It is up to you to decide what you would like to implement for your wellness strategy. 

I am connected with a complete network of nutrition consultants, wellness experts, and health practitioners. If I don't have an answer for you, I will find someone that does.
